Care Manager I

 We are seeking an experienced Care Manager I who will provide community-based services to live quality independent lives in the community. The Care Manager I will work with a dynamic team of professionals who provide innovative wrap-around services including, care management, occupational therapy for fall prevention, mental health counseling, caregiver support, volunteer friendly visitors and home-based enrichment programs. Care management services are provided to individuals of all income, religious, cultural, and socio-economic backgrounds.

Holocaust Survivor Care Manager

We are seeking an experienced Social Worker/Care Manager who will provide community-based services to support holocaust survivors to live quality independent lives in the community. The Care Manager will work with a dynamic team of professionals who provide innovative wrap-around services including, care management, occupational therapy for fall prevention, mental health counseling, caregiver support, volunteer-friendly visitors, and home-based enrichment programs. Care management services are provided to individuals of all income, cultural, and socio-economic backgrounds.

Clinical Supervisor

The Clinical Supervisor provides supervision and oversight to care management staff across programs to ensure high quality client centered care and services. The Clinical Supervisor maintains high-quality, ethical, person-centered services rooted in Jewish values of welcoming the stranger, repairing the world, and treating every person with respect and dignity, as well as in the core principles of social work practice.

 CalAIM Revenue Cycle Biller 

The CalAIM Revenue Cycle Biller is responsible for coordinating billing, data submission, and reimbursement processes for California Advancing and Innovating Medi-Cal (CalAIM) services, with a focus on Enhanced Care Management (ECM) and Community Supports (CS) programs. This role works closely with contracted Managed Care Plans (health plans) that process claims on behalf of the organization. The position ensures accurate service data submission, timely payment reconciliation, and compliance with Medi-Cal and payer requirements, while supporting overall revenue integrity and audit readiness.
