Home Care - Care Manager

Our Home Care - Care Manager manager is responsible for coordinating and overseeing in-home care services for clients, ensuring they receive safe, high-quality, and individualized support. This role involves conducting assessments, developing care plans, monitoring client progress, and collaborating with healthcare providers, caregivers, and families to achieve positive health outcomes.
