Older Adult Care Manager II

 We are seeking an experienced Social Worker/Care Manager who will provide community-based services to live quality independent lives in the community. The Care Manager will work with a dynamic team of professionals who provide innovative wrap-around services including, care management, occupational therapy for fall prevention, mental health counseling, caregiver support, volunteer friendly visitors and home-based enrichment programs. Care management services are provided to individuals of all income, religious, cultural and socio-economic backgrounds. This is a hybrid position. 

Recruiting & Staffing Specialist 

The Caregiver Recruiter Staffing Specialist is responsible for recruiting onboarding scheduling and supporting caregivers to ensure high quality compliant and compassionate care. This role manages the caregiver lifecycle including sourcing hiring training and retention while maintaining accurate records and compliance with state home care regulations. The position serves as a primary point of contact for caregivers and works closely with operations and care management teams to ensure proper caregiver client matching coverage and ongoing support in a fast-paced home care environment.
