Community Health Worker (Russian Language Required)

 Under the supervision of a Care Manager, the Community Health Worker provides essential administrative, logistical, and direct-support assistance to facilitate the delivery of care management services to CALAIM authorized members. The CHW supports the care management team in coordinating community-based services, conducting outreach, processing referrals, and ensuring clients receive timely and dignified access to the programs and resources they need. This role requires compassion, attention to detail, strong organizational skills, and the ability to work effectively with a diverse population. 

Care Manager I

 We are seeking an experienced Care Manager I who will provide community-based services to live quality independent lives in the community. The Care Manager I will work with a dynamic team of professionals who provide innovative wrap-around services including, care management, occupational therapy for fall prevention, mental health counseling, caregiver support, volunteer friendly visitors and home-based enrichment programs. Care management services are provided to individuals of all income, religious, cultural, and socio-economic backgrounds. 

Care Manager II (Russian Language Preferred)

 We are seeking an experienced Care Manager II who will provide community-based services to live quality independent lives in the community. The Care Manager II will work with a dynamic team of professionals who provide innovative wrap-around services including, care management, occupational therapy for fall prevention, mental health counseling, caregiver support, volunteer friendly visitors and home-based enrichment programs. Care management services are provided to individuals of all income, religious, cultural, and socio-economic backgrounds. 

Director of Holocaust Services 

The Senior Manager of Holocaust Services holds day-to-day stewardship of JFS Silicon Valley's Holocaust survivor services portfolio, including its Claims Conference contract, the State of California Holocaust Survivor Assistance Program (HSAP) allocation, care management, financial assistance, and emergency assistance functions. This role also manages the agency's homecare vendor contract with Institute on Aging (IOA), and maintains the agency's emergency assistance partnership with Kavod SHEF, ensuring survivors and other eligible clients receive timely, high-quality, dignity-centered support.
