Under donation amount, check the box next to “Dedicate my donation in honor or in memory of someone.” Then select “In honor of” or “In memory of.” Add their name in the “Honoree” fields. If you’d like to send anyone a notification of your gift, use the “Recipient” name fields.

Checking with your supervisor or payroll department. 2. Obtain Matching Gift Form: If your employer does offer a matching gift program, request a matching gift form from your HR department. This form is usually available online or can be obtained in hard copy. Complete the Form: Carefully fill out the matching gift form, providing all necessary information. This typically includes details such as: The name and address of the nonprofit organization (in this case, Jewish Family Services of Silicon Valley).The date and amount of your donation. Any additional required information about the organization (our Tax ID # is 94-2536452). 3. Submit the Form: Return the completed form to your HR department. This may involve: Sending the form via email. Uploading it through an online portal.Submitting a hard copy to the HR office. 4.
