JFS Silicon Valley Wins County Contract Expanding Services For Older Adults and Adults with Disabilities

We are pleased to announce that Santa Clara County has awarded JFS a new contract to run the Community Living Connection (CLC) program. This is a significant expansion of our aging services.
CLC provides care management and system navigation for older adults (60+) and adults with disabilities at risk of unnecessary institutionalization. The program connects clients to housing, benefits, and community services so they can remain safely at home.
Who qualifies?
Santa Clara County residents 60+ years old or adults with disabilities who are at risk of hospitalization or hospitalized in an acute or skilled nursing facility but able to continue living independently in their community with proper resources and support.
Why it matters?
This vital program prevents people from facing costly nursing home or hospital stays and enables them to continue living independently in their community.
